Release of Liability -

I acknowledge that this activity involves physical movement and risk.

I am aware that some of these risk include but are not limited to those caused by terrain, facilities, temperature, weather, conditions of participants, including but not limited to participants, volunteers and activity coordinators. I certify that the participants is in good health and capable to participate in this activity with the UIT Recreation Department. I understand the UIT Recreation Department staff and volunteers shall be free from any and all Liabilities, claims, injuries, damages or theft to anyone during this activity. Participates at their own risk.

Media Release-

I give my permission to the UIT Recreation Department to use my name or my child's name, photographs, video's or any likeness for publicity and the use of statesments made by or attributes the UIT Recreation Department for this event or similar evenrts for the use of promotions and grants the UIT Recreation Department to all copy rights without any compensation, I understand that signing below release the UIT Recreation Department from any Financial or legal responsibility the use of media and promotional material.

Medical Release:

Option 1:

I DO NOT want myself or child seen or have any medical attention unless the emergency contact is contact first.

Option 2:

I DO WANT myself or child to have minimum attention while staff is trying to get a hold of the emergency contact

Option 3:

I DO WANT myself and child to have full medical attention or what is necessary to provide to care that is needed while staff is trying to contact the emergency contact.

NOTE: Parents/Guardian we understand that you,may be worried about whether or not we get the applications.

Our staff DOES get your childs application please rest assured that once you submit your application it is complete and is being enterd into our system and into the sport that they wanted to be signed up in.
